Please note: We have specific forms you will have to sign and date at the time of service. 

YOUR FIRST VISIT

 

​OFFICE ARRIVAL TIME

       

  • ​If you have filled out the paperwork, we ask that you arrive at least 10 minutes prior to your appointment.  There 

 

 

 

      are some forms that you must sign in the office.​

 

  • If you have  NOT  filled out the paperwork prior to your visit, we ask that you arrive at least 30 minutes prior to your appointment. 

 

WHAT TO BRING

 

  • You MUST bring a copy of your insurance card & photo ID for identification and billing purposes

 

  • If you have seen a physician for this condition in the past, please bring any and all records; inclduing any x-rays, MRI or CT Scans

 

  • Any foot/ankle supports, braces, shoes or orthtotic inserts; customized or over the counter
